case-managementDescription: The Case Management team visits clients in their homes in order to determine eligibility for in-home services. Through the intake process, a needs assessment is completed, and appropriate referrals are made for services.

Case management for a caregiver may also include counseling and educating the caregiver on options that focus on the needs of the caregiving role.  See Caregiver Support.

Eligibility for PGCSC-Funded Case Management: To be eligible, clients must be:
  • Individuals 60 years of age or older,
  • Individuals 18 years of age and older with physical disabilities,
  • Caregivers of persons age 60 or older, or grandparents or older relative caregivers (55 and over) of children (under 19 years of age)
