Description: PGCSC-funded in-home services that help clients to remain at home include:
- Housekeeping: Provides help with activities related to routine home maintenance tasks.
- Home Health Aide: Provides health maintenance, on-going health treatment, and health monitoring.
- Personal Care: Provides limited assistance with bathing, dressing and grooming.
- Visiting Nurse: Provides skilled nursing services in the home.
- Respite: Provides short-term care and supervision at home or in a group setting to relieve the client's usual caregiver.
- Home-Delivered Meals: Delivers nutritious meals to eligible homebound persons.
Eligibility for PGCSC-Funded In-Home Services: To be eligible, you must be:
- At least 60 years of age and functionally impaired, or
Disabled, 18-59 years old, and have a physician-diagnosed physical or mental impairment that substantially limits your ability to perform activities of daily living.
Cost: Donations are greatly encouraged, needed, and appreciated.
