PGCSC Services: Behavioral health services provided by PGCSC include GateOpeners, outreach, prevention services, early intervention, counseling, and information and assistance for seniors, caregivers, and family members.
GateOpeners: GateOpeners is a structured community outreach, education and mobilization initiative that targets key individuals and organizations in the community who come into everyday contact with Seniors. Education is provided to enable these individuals and organizations to identify risk behaviors and to make referrals to the appropriate services. Prevention Services: Prevention services are provided by PGCSC to individuals at risk for serious behavioral health problems, as well as persons experiencing situational stress due to temporary life pressures. There is no income requirement or fee for these services, Donations are accepted.
Eligibility
for PGCSC Prevention Services - Clients generally
must:
- Be 60 years of age or older, or physically
disabled and 18 years of age or older, and
- Be a grandparent raising a grandchild and 50
years of age or older
- Be a caregiver of someone that is 60 years of
age or older
Ongoing Counseling and Other Mental Health Services: Ongoing in-home counseling and other mental health outpatient services are provided by PGCSC for persons with medically necessary mental health needs, including psychiatric medications and ongoing Individual, family or group counseling. Specializes in geriatric behavioral health services (life changes associated with aging, substance/medication abuse, addiction, depression, isolation, leisure development, grandparent and adult caregiver issues, and long-term care planning.)
Eligibility for PGCSC Counseling Services: Clients generally must:
- Be 60 years of age or older, or physically
disabled and 18 years of age or older
- ALTCS or AHCCCS members must meet eligibility
criteria, or
- Private pay also accepted. .
Support Groups - Support groups provide information and emotional support for seniors, caregivers and others. For a description and list of Support Groups, click here.
