Description: The Case Managers visit clients in their homes. They determine the person’s eligibility for in-home services, assess needs, and make appropriate referrals for needed services. A PGCSC-funded service.
Eligibility for PGCSC-Funded Case Management: To be eligible, the client must be:
- At least 60 years of age and functionally impaired, or
- Disabled, 18-59 years old, and have a physician-diagnosed physical or mental impairment that substantially limits your ability to perform activities of daily living.
Availability: Available in Pinal and Gila counties.
